Tumescent liposuction is an advanced technique, performed entirely under local anesthesia is performed. This involves the injection of a great deal of anesthetic fluid into the area to be treated, causing the fat tissue to firm up and soften the fat, so to speak. This allows the fat to be removed more safely, easily, beautifully and with less tissue damage. Tumescent liposuction has many advantages over traditional liposuction.

Safest technique(1):
Partly because the treatment is performed under only local anesthesia, the risk of serious physical complications is virtually zero (in our team in > 5000 treatments have not yet occurred).

More beautiful results (sculpting):
The tumescent fluid enlarges and evens out the treatment area, allowing the doctor to work much more precisely. Also, local anesthesia allows patients to change positions during treatment. As a result, more beautiful, even results can be achieved.

Faster recovery:
Because less damage is done to the tissue and only local anesthesia is used, recovery after tumescent liposuction is generally faster and less painful. Patients can go home the same day and resume their daily activities within a few days.

Lifting effect/ skin tightening:
due to the injected fluid and the use of thin vibrating cannulas, the connection of the skin to the muscle is not damaged. This prevents the skin from sagging. In addition, the tissue is activated which causes the skin to contract even more (elevator).

A skin correction is therefore rarely necessary with tumescent liposuction.

To perform liposuction painlessly, anesthesia is, of course, necessary. There are several methods for this. For example, you can go into full anesthesia, receive an epidural or anesthesia through an IV, or only be numbed locally (local anesthesia).

Scientific research shows that liposuction treatment under anesthesia has greater risks than local anesthesia alone (1).

In other words, the more awake and mobile the patient is during treatment, the safer. Thereby, you also get nicer results if a patient is mobile and can assume different positions during the treatment.

Several new liposuction techniques have been developed in recent years, often presented as innovative and promising. Consider, for example:

  • Ultrasound-assisted liposuction (UAL): in which sound waves loosen fatty tissue before it is suctioned out.
  • Laser liposuction: in which a laser beam heats and breaks down subcutaneous fat.
  • Water jet liposuction: in which a powerful jet of water flushes out the fat.
  • Power-assisted liposuction (PAL): in which a vibrating handpiece vibrates the fat loose.

These techniques sound innovative, but the scientific evidence for better or safer results is limited. Indeed, some methods actually carry additional risks, such as burns, scarring or damage to surrounding tissue.

Therefore, at Lipology Clinic, we deliberately choose tumescent liposuction with fine vibrating micro-canulas. This is the most researched, proven safe and effective method. This technique offers smooth and natural results, minimal tissue damage, shorter recovery time and additional skin tightening, without the risks of laser or ultrasound.

Yes, in many cases, tumescent liposuction is also possible in overweight people. Because this method is performed under only local anesthesia (without anesthesia), the conditions to be treated are less strict than with traditional liposuction under anesthesia. Even patients with a BMI over 30 can sometimes be safely treated, provided that medical history, condition and any medication allow it. This is always carefully assessed during the intake interview.

However, it is important to note that liposuction is not a weight-loss method. The goal of the treatment is contour improvement: the removal of stubborn fat deposits that do not disappear through diet or exercise. So liposuction does not provide major weight loss, but it does improve body shape and comfort.

The treatment is most effective in people who are already close to their healthy weight but still have specific problem areas, such as abdomen, hips, thighs, arms or knees. In more overweight patients, liposuction can still make a big difference in reducing symptoms, improving mobility and experiencing more self-confidence.

Yes, there are several treatments today that are presented as alternatives to liposuction, removing fat without surgery. These techniques fall under the collective term lipolysis and aim to damage or break down fat cells. Examples include:

  • Cryolipolysis: the local freezing of fat cells with a cold applicator on the skin.
  • Ultrasonic lipolysis: the “pulverization” of fat cells through sound waves delivered to the skin via a device.
  • Injection lipolysis: dissolving fat by injecting a chemical substance under the skin.
  • Laser lipolysis: the melting away of fat cells using a laser beam under the skin.

While these methods sound appealing because they are nonsurgical, they also have obvious limitations. The results are usually much less effective and predictable than with liposuction. At best, they are suitable for treating very small fat deposits, such as under the chin or a small roll near the abdomen. For larger areas, such as abdomen, legs, hips or arms, liposuction is by far the most reliable and permanent solution.

In addition, these nonoperative treatments also carry risks, such as burns (with cryolipolysis), hard hump formation, skin irritation or allergic reactions. Effectiveness varies greatly from person to person and often multiple sessions are required with limited visible improvement.

This is why liposuction with the tumescent technique is still considered the gold standard: safe, predictable and with lasting results.

There is no strict age limit for liposuction. Even later in life, tumescent liposuction can be a safe and effective treatment provided you are in good health. The most important condition is that you are fit enough for the procedure and have no medical contraindications.

In older patients, there is often the question of whether the skin still has sufficient retraction after the removal of fat. Although skin elasticity decreases somewhat with age, we see that the skin can often recover surprisingly well even in old age. Thanks to the tumescent technique, which uses thin vibrating cannulas and preserves the skin structure, a natural lifting effect also occurs. Because of this, an additional skin correction is not necessary in most cases.

Step-by-step course of liposuction:

  1. Reception and preparation
    You will be received by our regular team. Medical checks follow and the doctor carefully marks the area to be treated.
  2. Injection of tumescent fluid
    Through fine needles, we inject a special anesthetic (tumescent fluid). This loosens the fat and numbs the area. This process usually takes about an hour.
  3. Incorporation time
    The fluid needs time to take effect properly. You will rest for at least half an hour (often a little longer), so that the anesthetic has optimal effect and the fat can be removed more easily.
  4. Fat removal

Tumescent liposuction is known worldwide as the safest form of liposuction. Because the treatment is done under local anesthesia, the risks are many times lower than liposuction under anesthesia. Serious complications are very rare and have never occurred in our team, with thousands of treatments experience.

As with any medical procedure, temporary side effects may occur, such as:

  • Swelling and bruising
  • Temporary numbness or tingling of the skin
  • Itching or tension of the skin during recovery
  • Side effects due to anesthetic or antibiotic (which we always screen for beforehand)

Yes, fluid accumulation and swelling are normal reactions of the body after liposuction. In the first few days this is due to the anesthetic fluid, then wound fluid. This is temporary and largely disappears in the first few weeks. With tumescent liposuction there is no permanent damage to blood or lymph vessels, so that the fluid is drained away by itself. This makes this technique particularly suitable for people with lipedema.

To support the recovery process, you will be fitted with a pressure suit after treatment:

  • You will wear bandages with a pressure suit over them for the first 24 hours. After that, you may shower and remove the bandage.
  • You then wear the pressure suit day and night for at least 2 weeks. This promotes fluid drainage, reduces swelling and supports skin recovery.
  • Unlike traditional liposuction (where 6 weeks of pressure garments are often advised), recovery time with tumescent liposuction is shorter due to minimal tissue damage.
  • Some clients like to wear the pressure suit longer, such as during the day. This is not a problem and can provide additional support for recovery.

After liposuction, swelling always occurs due to wound fluid and the insertion of the tumescent fluid. To support this recovery process, you will wear a pressure suit day and night for the first 2 weeks.

Some clients choose to additionally undergo manual lymph drainage (massage by a specialized edema therapist). This massage can help to:

  • Stimulate the lymphatic system and drain excess fluid faster.
  • Making the skin more supple and reducing adhesions.
  • Making recovery more pleasant.

Massage is usually possible from 1 to 2 weeks after liposuction, once the initial sensitivity has subsided. Important to know: massage is not a requirement for a good end result. Wearing the pressure suit in combination with sufficient exercise is already sufficient for a successful recovery. Above all, massage can make the process more comfortable.

How soon you can return to work after liposuction depends on the type of work and your personal recovery. Thanks to the tumescent technique, recovery is usually smooth. The guidelines are:

  • Office work or light duties: often possible again within a few days.

Yes, the result of liposuction is basically permanent, because the removed fat cells do not return. This is because your body does not produce new fat cells. However, the remaining fat cells can grow larger if you gain a lot of weight in the future. Unfortunately, liposuction, even for lipedema, is uninsured care. Therefore, requesting an “authorization” by the physician, as is often advised to lipedema patients by the health insurance company over the phone, is pointless. Indeed, the doctor’s request will ultimately be rejected by the health insurer based on the opinion of the Dutch Health Care Institute (ZiN).
